Hair Loss and Iron: The Clinical Connection

Table Of Contents

Hair thinning or excess shedding? Low iron could be a factor — even if your blood tests look normal. What ferritin levels really mean for your hair health.

Why Iron Matters for Hair Health

Iron is required for oxygen transport, cellular energy production and DNA synthesis. Hair follicles are among the most metabolically active structures in the body. To sustain continuous growth, they depend on a reliable supply of oxygen and nutrients. When iron availability is reduced, the body prioritises vital organs such as the brain and heart, and hair growth becomes a lower physiological priority.

Iron is also required for enzymes involved in cell proliferation within the hair follicle. When iron stores fall, these processes slow, increasing the likelihood of premature hair shedding as well as leading to poor quality hair.

The Hair Growth Cycle and Iron Deficiency

Hair grows in a continuous cycle with three key phases:

  • Anagen (growth phase): lasts several years and determines hair length and density
  • Catagen (transition phase): a brief period of follicular regression
  • Telogen (resting and shedding phase): hair is shed before the cycle restarts

Iron deficiency is strongly associated with telogen effluvium, a condition in which an increased proportion of hairs prematurely enter the telogen phase usually triggered in the main by stress. This typically presents as diffuse thinning rather than patchy hair loss and is often noticed as excessive hair shedding especially noticed during washing or combing.

Hair Loss Without Anaemia

Crucially, iron-related hair loss can occur before haemoglobin levels fall and before iron deficiency anaemia is diagnosed. Ferritin, a marker of iron stores in health, is often more relevant in the assessment of hair loss than haemoglobin alone. When the body is in an inflammatory state, ferritin can be raised and thus unreliable as a marker of iron stores and so specialist interpretation of full iron panel is important.

Critical Insight on Ferritin Levels

It is widely accepted that hair growth can be affected at ferritin levels below 80 ng/ml. Ferritin should be maintained at 80 to 100 ng/ml for 2 consecutive years to optimise hair (re)growth.

Dermatology research supports this link. A review published in the Journal of the American Academy of Dermatology reported significantly lower ferritin levels in women with diffuse hair loss compared with controls, despite normal haemoglobin levels. Similarly, studies in Clinical and Experimental Dermatology have shown that women with telogen effluvium are more likely to have depleted iron stores, reinforcing ferritin as a key marker in hair loss assessment.

This helps explain why individuals may be reassured that their blood tests are “normal”, yet continue to experience ongoing hair thinning.

Who is Most at Risk?

Iron-related hair loss is more common in certain groups due to increased iron loss, reduced intake or impaired absorption.

Regular menstrual blood loss places women at higher risk of gradually depleted iron stores, even when periods are considered normal.

HMB is a leading cause of iron deficiency and may trigger diffuse hair shedding before anaemia is detected.

Diets low in bioavailable iron, particularly those relying on plant sources, can lead to insufficient iron intake over time if not carefully balanced.

Increased iron losses through exercise, inflammation and sweat mean athletes often require higher iron intake to maintain normal stores.

Disorders affecting the gut, previous gastric surgery or long-term acid-suppressing medication can limit iron absorption, prolonging deficiency.

Illness, surgery, pregnancy, postpartum changes or significant stress can increase iron demand and trigger hair shedding, particularly when iron stores are already low.

Can Iron Replacement Support Hair Regrowth?

Correcting iron deficiency can support the return of hair follicles to the anagen (growth) phase, but improvement is gradual. Hair growth occurs slowly, and shedding may continue for several months before noticeable regrowth is seen usually starting with baby hairs in the front hairline & crown.

Oral Iron Supplementation

For some individuals, oral iron supplementation is sufficient. However, absorption of oral iron can be limited by gastrointestinal side effects and thus compliance.

Intravenous Iron Therapy

In cases of significant deficiency, poor tolerance, or persistent symptoms despite oral supplementation, intravenous iron should be considered as directed by experienced doctors.

Comprehensive Assessment

Any iron supplementation should be as a result of accurate interpretation of blood results including a full iron panel.

A Holistic, Medical Approach

Hair loss is rarely caused by a single factor. Hormonal balance, thyroid function, nutritional status, stress levels and overall health all play important roles. Iron deficiency should be considered as part of a broader clinical assessment rather than in isolation.

Key Take-Home Points

  • Iron deficiency can contribute to hair loss even without anaemia
  • Low ferritin levels are commonly associated with diffuse hair thinning and telogen effluvium
  • Ferritin should be maintained at 80 to 100 ng/ml for 2 consecutive years to optimise hair (re)growth
  • Research shows women with hair loss often have lower iron stores than unaffected controls
  • Certain groups are at higher risk due to iron loss, intake or absorption issues
  • Correcting iron deficiency can support hair regrowth, but results take time (6 months) and require medical oversight
